For pediatricians, regional center coordinators and therapists. The route, the delays, and what to tell a family before you send them.

The short answer
After a referral, a good provider talks with the family first, then interviews the parent and assesses the child. A report goes to the health plan or regional center for approval, which is where most of the wait happens. Then a therapist is matched to the family's week. Tell families the wait is mostly paperwork, not that they were forgotten.
Key takeaways
- Most of the wait is funding approval, not clinical.
- A parent or guardian needs to be present for sessions. Say so at referral.
- In California, state-regulated plans must cover medically necessary behavioral health treatment (SB 946), and Medi-Cal covers it through EPSDT.
- Refer even before there is a diagnosis.
You refer a family and then, usually, you hear nothing. This page is about what happens in that gap, so you can tell a family what to expect and know when something has gone wrong.
The route, in order
A conversation first. Before any paperwork, someone should talk to the family about what is actually hard right now. A provider who opens with forms is telling you something.
An interview with the parent, then assessment of the child. The parent interview comes first for a reason. Direct assessment follows across several appointments.
A report, then funding approval. The behaviour plan and the goals go to the health plan or the regional center. This is where most of the waiting lives, and it is the part neither you nor the family can hurry.
Scheduling. Matching a therapist to a child and to a family's actual week is its own constraint, and it is often what decides the start date rather than clinical readiness.
What to tell a family before you send them
That the wait is mostly administrative rather than clinical, so silence does not mean they have been forgotten.
That they should ask any provider what happens when a goal stops moving. The answer tells you most of what you need to know about a clinic.
That a parent or guardian will need to be present for sessions. Families who find this out in week one sometimes cannot make it work, and it is far kinder to raise it at referral.
That in California, SB 946 requires state-regulated plans to cover medically necessary behavioural health treatment, and Medi-Cal has its own route through EPSDT that is often broader. A plan saying no on the first call is not always a final answer.
When a referral is not the right one
ABA is not the answer to everything, and a family sent to the wrong service loses months.
If the presenting need is primarily communication, a speech and language assessment may need to come first or run alongside. If it is primarily sensory or motor, occupational therapy. If a family is in crisis this week, they need a crisis response, not an intake process that runs for weeks.
A provider worth referring to will tell you this rather than take the referral.
What we do with one
We contact the family, in English or Spanish, and we handle the coverage call rather than sending them to do it. If the family agrees, we confirm back to you that contact was made and whether care started.
If we are not the right fit, because of caseload in that area or because the need sits outside what ABA treats, we say so and give you names. We would rather pass a referral on than hold it open.
No diagnosis yet
Refer anyway. Much of the value in an early conversation is explaining how the diagnostic route works in California and roughly what each step involves. Families lose months in that maze, and a short call often saves them one.
Questions people ask
How long does it take to start ABA after a referral?
It varies. Most of the wait is funding approval from the health plan or regional center, then matching a therapist to the family's schedule. Silence usually means paperwork, not that the family was forgotten.
Does a child need an autism diagnosis to be referred for ABA?
No. Refer anyway. An early call can explain how the diagnosis route works in California and what each step involves, which often saves families months.
Does insurance cover ABA in California?
Often, yes. SB 946 requires state-regulated health plans to cover medically necessary behavioral health treatment, and Medi-Cal covers it through EPSDT. A no on the first call is not always final.
When is ABA not the right referral?
If the main need is communication, a speech and language assessment may need to come first. If it is sensory or motor, occupational therapy. A family in crisis this week needs a crisis response, not an intake process.

This is what working with us is like
Everything on this page is how we actually work: one change at a time, written down, kept or dropped on what the notes say. If you would like that applied to your own child rather than read about, the first call costs nothing.
